care management and program administration services to the aged,
blind and disabled Medicaid population of central and southern
Georgia. AmeriChoice capitalizes on more than 20 years of
expertise in the design, implementation, and management of
government health care programs to offer an array of services from
strategic consulting to risk-sharing health programs for specialty,
complex needs populations served in a fee-for-service environment.
Our approach to the
Georgia Enhanced Care (GEC) Program emphasizes two distinct and
unique factors differentiating our program from others. First, we
provide a comprehensive set of expertise and resources to support
this program, including our demonstrated success working with
the highly complex and challenging needs of the Medicaid
population. Second, we bring a commitment to achieve program goals
that encompasses innovation and flexibility, allowing the State to accomplish its
stated objectives through a highly inventive program.
Georgia Enhanced Care Program Fact Sheet
Charmaine Martin Heard, MD, Medical Director.
Dr. Heard brings over ten (10) years of experience in serving public
health programs and is currently the Medical Director for AmeriChoice’s Georgia-based disease management program serving the
aged, blind, and disabled Medicaid members of Southern Georgia. Dr.
Heard completed her Bachelor’s of Science degree at the University
of Massachusetts and Doctorate of Medicine at Brown Medical School
in Providence, Rhode Island. Dr. Heard fulfilled her internship and
residency programs for Internal Medicine at the Boston City Hospital
in Boston, Massachusetts.
Gay Hoffman, RN, Director of Operations.
As the Director of Operations for AmeriChoice’s contract to provide
disease management services for Southern Georgia’s aged, blind, and
disabled Medicaid members, Ms. Hoffman brings over ten (10) years of
direct patient care as well as health care program administration
experience. She has served in leadership positions with increasing
responsibility and offers a community-focused approach to program
planning. Ms. Hoffman completed her nursing degree at the Florida
Community College in Jacksonville, Florida.
